Survey Forms:

• TPII Knowledge, Attitudes,and Behavior Student Survey Instrument, English version.
• TPII Knowledge, Attitudes, and Behavior Student Survey Instrument, Spanish version
• TPII Teacher Inservice Assessment

TPII survey instruments (or "forms") have been tested for validity and are based upon risk and protective factor research.
Student instruments (both English and Spanish versions) are four pages and contain 38 questions.
The teacher instrument is 2 pages and contains 18 questions.
Both English and Spanish forms are available only in quantities of 100.

Determine sample number (see TPII replication Guidelines for recommended formula).
Add 10% total to allow for extras.
Determine type of shipping required.
Place order one month in advance of data needed

Scanning Services

Scanning Services provided by Research and Educational Services.

Implement Survey
Determine types of records desired (such as by grade level, campus, district, and/or  composite).
Indicate on Report Order Form provided by Development Solutions.
Enclosed  Report Order Form and ship completed student and/or teacher forms to Research and Educational Services.

Survey Findings Written Report
This option is only available to districts/organizations who use Development Solutions and Research and Educational Services to provide scanning services.

The Student Profile is provided in two different formats. The Teacher Inservice Findings is a text-only format.
One hard copy Black/White master provided of:

• District specific TPII Risk/Resiliency Profile (Student report showing percentages of students' responses)- 9 pages
• District specific TPII Graphical Data Assessment (Bar graphs of students' responses)- 27 pages.
• Narrative Summary of TPII District specific Teacher Inservices Assessment Findings -3 pages

Color Presentations Transparencies
(graphs of findings)

Enhance Chart in vivid color, suitable for a ready-made presentation on an overhead project of TPII district findings.

Statewide Norms

Statewide averages of all TPII data as processed through Research and Educational Services

Year-to-Year Comparison Written Report

This option is designed for districts that have used TPII for several years and would like a comparison of findings. Report format includes graphs as well as text.

On-site Technical Assistance

Assistance can be tailored to the needs of the district/organization.
Options include assistance with developing an implementation plan and/or using data finding to better plan prevention programs. It will be provided by one of the original TPII development team members.

Work with Development Solutions to determine assistance needs.
Schedule a date/time for technical assistance visit.
Determine who needs to be involved in the TA session and inform participants.

On-site Presentations of Findings

Audio-visual presentation of district's findings of approximately 1.5 hours in length ( including time for questions/answers). Presenter will be one of the original TPII development team members.

Work with Development Solutions date and schedule presentation.
Invite attendees and/or advertise presentation to target audience.
